You will also need your health insurance information (if you plan to pay with health insurance). Self-pay clients (clients without health insurance) or clients who would rather not notify their insurance company will have options here as well.

If you are an existing client who has seen one of our therapists within the last month or on an ongoing basis, please use this form to schedule your appointment. Please note: You must have been seen in our 910 S Rogers St Clinic after Feb 1, 2022 to be an existing/ current client. If you have seen one of our Clinicians at a different location prior to Feb. 1, 2022, then please complete the NEW CLIENT form – otherwise, please continue to the CURRENT CLIENT form .
Our Facilities are Accessible
